"Exception Not Found" When Approving

Symptoms: An Approver clicks “Approve” on an exception request but receives a “not found” error.

Cause: The exception entity may have been deleted, or the exception has already been processed (approved, revoked, or expired) by another session.

Fix: Refresh the case detail modal. If the exception is not shown, check the Audit Log for the case to see what happened to the exception entity.
